Instructions1. You MAY nominate more than one person for Alumni of the Year. Each nomination must be a separate submission.
2. Review and address all required criteria.
3. Select the award category for the individual/organization being nominated.
4. Supporting narrative is limited to one page. Keep it brief.
5. Application must be received by May 25, 2012 and becomes property of Leadership Fresno. A copy will be automatically emailed to you for your records.

Nominations not being submitted through this online system may be mailed, faxed, or emailed. (A PDF form is available on the previous page).
